Membership Form

Please Note:

  1. All new members – the preferred method of applying for membership is via join.sls.com.au (as per the link on the ‘Membership’ page). For those who prefer the paper form, please download the Application Form at the link below and ensure you use block letters and write clearly (so that the details/spelling will be entered correctly in the Membership Register).
  2. All existing members are asked to renew and pay online, as this greatly assists the Club’s volunteer administrators.

New Members must present the ‘Application for Membership’ form, along with proof of ID and payment of fee to a committee member (who will record the details and pass these on to the Registrar).

Existing Members need only complete the online Application for Membership/Renewal and pay their  fee (also online).

Working with Children Check Form

Adult members who work closely with children for 5 days or more per year and don’t have an exemption, will be required to provide a “Working with Children Check” number and validity dates. The Club will verify the member’s status with the Office of the Children’s Guardian and attach the Clearance to the member’s record in Surfguard. NB: The WWCC is free for volunteers, must be renewed after 5 years and is portable. i.e. Can be used for any organisation for which an adult is engaged in child related work as a volunteer.

Forms Relating to Member Injury

If you have suffered an illness or injury while performing authorised surf lifesaving activities, Workers Compensation may be payable under the Workers Compensation (Bush Fire, Emergency and Rescue Services) Act 1987.
